INTERNATIONAL FORM

INTERNATIONAL SAMPLE INSTRUCTIONS

International samples require specific permitting and paperwork to ensure samples arrive in a timely manner. For this reason, all requests for submission should call (814-863-0918) or email (turfpestlab@psu.edu) PRIOR TO SENDING samples to make sure correct procedures and permits are followed.

A. Contact our lab via email prior to sending any samples

A.1. Contact us [email: turfpestlab@psu.edu] first to obtain pre-approval and accompanying USDA shipment labels and our permit or request the USDA labels directly from Dr. Louis Bengyella (turfpestlab@psu.edu).

B. Collect Samples

B.1. Submit a Cup Cutter-sized sample (4” diameter x 3” depth) that includes half symptomatic and half healthy turf. 

B.2. Include a completed Diagnostic Form with your sample(s). These are available at turfpestlab.psu.edu or can be requested at TurfPestLab@psu.edu. 

B.3. Complete the form with as much information as possible and keep the form separate from the sample in a Ziploc bag.

B.4. Do not shipped samples collected 12 hours prior to courier shipment. Fresh samples are preferred. 

C. Preparing samples for shipment

C.1. Wrap samples in aluminum foil and pack tightly in a shipping container. DO NOT place samples in a plastic Ziplock.

C.2. Place wrapped samples in an unsealed/open box along with color copy of the USDA permit and labels provided (from A.1). (Labels MUST be printed in color or they will not past customs)

D. Shipping instructions

D.1. Place the box (from C.2) in Express Courier Shipping Box and affix a color copy of the USDA permit and labels provided (in A. 1) to the outside of this shipping box. (Labels MUST be printed in color or they will not past customs)

D.2. Ship overnight/express to the Turf Pest Diagnostic Lab address listed below

E. Cautions

1. All samples must be accompanied with a completed Diagnostic Form available at https://turfpestlab.psu.edu or can be requested at TurfPestLab@psu.edu.

2. Placed the completed Diagnostic Form in a plastic bag and place it in box 1 (in A.1.).

3. Samples are only received and processed on working days.
